I now participate with several commercial health insurances. Please see below. However, if I do not participate with your insurance plan, I am sure you will find my rates more than competitive.

Simple, affordable, & transparent rates.

1. New patient intake evaluation and medication management - $ 275 dollars. 

2. Follow up with medication management: Up to 30 minute visits - $ 125 dollars.

3. Independent psychological evaluation and detailed report for civil service employment denials and firearms applicants - $650 dollars.

4. Neurocognitive capacity psychological evaluations - $1200 dollars.

5. Refill of any schedule IV medication (1 month ONLY) is subject to a - $ 45 dollar charge.  

 

Testing:

• Genetic Gensight Testing is $150 dollars (However, sometimes your insurance will cover the lab expense).

• Any other lab monitoring needed to is included in the price.

 

Miscellaneous:

• Medical Marijuana Cards are $150 dollars. Annual renewal is $100 dollars.​​
